Instructions
There are four different ways in which you can complete the Extended Credit Agreement:

1. Fax (preferred)
Print out this form, complete the details requested, sign it and then fax it to us at:

2. Scan and email
If you have access to a scanner, the second option is for you to print, complete and sign the form by hand, exactly as with the fax option and then to use your scanner in order to create an image file (preferably as a gif). Then attach the image using the form on the Contact Us page.

3. Voice Recording
If the first two options are not available to you, then you can make a voice recording of your Extended Credit Agreement. Click here to find out more.

4. Post
Finally, you may mail the completed agreement to: CC Network Inc., Zekeringstraat 17, 1014 BM, Amsterdam, Netherlands.

Once we have received and verified the signed agreement we shall promptly remove the spending limit on your account and notify you by email that this has been done (to the email address you provided below). This should hopefully be done almost immediately. However, in the unlikely event that you do not receive this notification by email within a 12 hour period, please re-send the form.
